In those cases I would still imagine a neurotic desire to live up to a certain self image is the drive for doing these things. again their desire is to be something... that something drives their actions... it's still selfish...

It is just my opinion.
It is my personal belief that there are clearcut instances of "good and evil" behavior, that may have started out as instinct, but later evolved in us due to changing society.
Thousand of years ago, a mother may have died for her children to propagate the species. They were young and they would live on and have kids themselves, etc.
Now that our species is established, we no longer have a need to die for our children to save the planet. Instinct has turned into emotion for most of us.
On that note, I would die for my children and have no idea if they will later have children or not.

Your argument of "good and evil" is quite unfounded.
When a religious man chooses to serve another person, he is doing it with the express desire to make it to heaven. Is it evil that he has self interest, or is it good that he is helping someone for a "greater good"?
Good and Evil are relative, don't start throwing them around like you can define it. We all act in self interest; whether or not you choose to recognize it does not mean that it is "evil". On the contrary, self interest is what moves society to innovate.
