been feeling kind of empty since i was about 11, ive gotten use to it.

You have to find more sex partners and slowly acclimatise them to sadism.

After a longer period of not seeing someone get hurt or hurting someone personally (emotionally or physically , doesn`t matter) I start feeling extreme levels of anxiety. Chronic depression and feeling of emptiness.

It sounds to me that you use violence and sadism against others to supplant feelings of alienation and/or inferiority with perceived dominance. When you become unable to assert yourself in such a manner, your boosted ego slowly withers, and the anxiety that you sought to bury resurfaces itself.
