It takes a lot longer, but orgasming can be over other sensations and themes. 

The key is in letting go. Sexual trance can help a lot with that, which is why porn can be so effective, but it's not necessary. Most of what people get off to during masturbation is associative, it's what we've connected with the act over numerous times to the point of it imprinting on the mind as something more automatic. Through the same idea you can get off to pretty much anything if you set the right conditions for it and do it enough times.
