It isn't "about" one thing. It's strength is built on mutuality. Otherwise, it just has the character of something unrequited, which makes whatever it's "about" an oxymoron.
Exactly, it's about synergy. If you're getting nothing out of the other person there's nothing that gives it value.
Delora's loyalty is admirable. She will be there for a friend because she considers it her responsibility.
Would they be there for her? There are professionals whose business it is to care for people. They have the same sense of responsibility. Are they everyone's friend? You could argue that these professionals are being compensated financially for their services, so that their motivations could be far from altruistic. Even minus that sort of incentive, all that would mean is they are a Good Samaritan, which in some ways isn't much more than the social contract we're assumed to live by. This says more about Delora's ethics than the character or validity of any particular relationship.
