A good fantasy world for me looks like a game world. Defeat monsters and level up as an adventurer. Magical powers. Terrifying foes, of all kinds. Breath-taking sights. Expansive and all-encompassing. All that you listed included, and more. The technology level of the world should be at medieval levels, augmented with magic. Demon lords to defeat. Undead armies to protect the kingdom from. Terrors from the void looming at every shadow.

The D20 campaign I've been running with the same high school/college group as when I started within a single world continuity that borrows from the entirety of our game history's been more than enough for me.
Every new campaign we do has the plot advance five to ten years into the future, allowing for carryover of past deeds to affect future runs.
What has it so entrancing for me is over how it constantly evolves, yet maintains a deeply rooted foundation of continuity that plays into their Knowledge(History) checks and the like.
A world that is like a large garden maze but as you make your way through the bushes change shape and the layout changes, but the exit is always in the same spot. I imagine this maze to look like the bush walls in the Queen of Hearts garden level of Kingdom Hearts.

Or maybe I could retire after all the adventuring is done and get a nice home on some secluded land. There would be a large mountain. A waterfall falling from the top into a river. A crystal cave beyond the waterfall. The home would be close to that surrounded by a grassy portion, and then a large and dark forest. With a portal to the city and back inside of the home, of course.

Large rose bushes, but in some areas butterfly pea flower, baby's breath, pussy willow, and some more treacherous areas would have brambles and thorn bushes. Sporadic White sycamores a humdred feet tall, the ground around them littered in fallen scragally branches.
